Consent to the tattoo procedure

I acknowledge by signing this agreement that I have been given the accurate and full information on the procedure of obtaining a tattoo and I give my full consent on the application of it. As the procedure involves minor breakage of the skin with a needle I understand the health risks, therefore I am obliged by law to declare any conditions that will affect this procedure.

Please read and tick the points below.

I am over the age of eighteen and have truthfully represented myself of age

I have not consumed drugs or alcohol in the last 24 hours

I have no heart conditions or blood clotting disorders

I do not have any conditions that compromise my immune system

I do not suffer from any blood borne disease includingHIV,AIDS, Hepatitis A,B or C

I do not have any allergies. If I do I have discussed this with my artist

I am not pregnant or breastfeeding

I do not suffer from any form of seizure causing conditions or prone to fainting or dizziness

understand that every care has been taken to ensure that this procedure has been carried out in a hygienic matter

I understand that after leaving the shop the aftercare and healing of this tattoo is MY OWN responsibility, not that of the artist, and that aftercare has been explained to me

I understand that reaction to pigment is still possible even after it’s healed

This is the design I have requested and any spelling/dates are correct

I understand that the finished tattoo can look slightly different from the original design

I consent to any action necessary to perform the tattoo
